City of Santa Ana <br />Scope of Work <br />Name of Organization Rebuilding Together Orange County <br />Name of Funded Program Safe at Home 2008-2009 <br />Annual Accomplishment Goal <br />I. Total number of unduplicated Households (Santa Ana and Non -Santa Ana Households) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />20 Households <br />II. Number of ONLY unduplicated Santa Ana Households to be served by the funded program, named above, during <br />the 12-month contract period. <br />20 Households <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Scope of Work: Rehabilitation of 20 single family, owner - occupied homes of low - income families in the City of <br />Santa Ana. Homes are assessed for actvities to maintain safe, livable conditions for household members who are <br />unable to provide for themselves either economically or physically. Typical work tasks include organizing <br />contractors and volunteers, obtaining materials and other resources to address improvement to exterior - interior <br />access areas, bathroom - kitchen modifications and including handrails, lighting, grab bars in baths and showers, <br />wheelchair ramps, household apppliance - heater repair, elecetrical - plumbing repair, roofing and painting needs. <br />Rehabilitation modifications and/or repairs enable homeowners to continue living in their Santa Ana homes in <br />safely, warmth and with dignity. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana households to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana households served each quarter.
