Healy <br />MITIGATED NEGATI~/E DECLAMATION <br />Pursuant to the Procedures of the City of Santa Ana for implementation of the California <br />Environmental Quality Act, the Environmental Evaluator has completed an Initial Study for <br />the project described below: <br />Project Number: GPA 2007-3, ZOA 2009-2 and ER 2006-142 <br />Applicant: Vinh Tran <br />Project Location /Address: 3417 West Fifth Street, Santa Ana, CA <br />Project Title /Description: Medical Office Building <br />The proposed project consists on the construction of a two story, 6,002 square foot <br />medical office building and related parking and landscape in compliance with <br />development standards for the general commercial zone. The site will provide 36 <br />parking spaces and have vehicular access from Jackson and Fifth Street. A zoning <br />ordinance amendment to the North Harbor Specific Plan (SP2) to change the <br />boundaries of the land use category map from Open Space to General Commercial <br />and a general plan amendment to change the land use designation from Open Space <br />to General Commercial are required. <br />And does hereby find: That although the proposed project could have a significant effect on <br />the environment, there will not be a significant effect in this case because of <br />revisions to the project and mitigation measures placed on the project, and agreed to <br />by the applicant, reduce each impact to below a level of significance. <br />Signature: Date: <br />Senior Planner <br />This determination is not final until adopted by the decision-making body or administrative <br />official, and a Notice of Determination is filed. <br />75A-21 <br />
