FAC/L/TY SERVICES <br />GENERAL MAINTENANCE AND ADDITIONAL INFORMATION <br />1. Remove all leaves, litter, weeds in hardscape, and all other debris from walkways, curbs, gutters, <br />and landscape areas, and dispose off- site. <br />2. Blow or sweep all above-mentioned areas after completion of mowing, trimming, edging, <br />weeding. <br />3. Inspect all landscape areas on a monthly basis for pest, rodent and disease problems, Control <br />by approved method whether chemical or mechanical, conforming to all safety rules and <br />regulations regarding the use of pesticides and chemicals as prescribe by the State of California. <br />4. Maintain all drainage systems free of siltation, litter or other collected debris at all times. A <br />complete check of the entire drain system will be done on asemi-annual basis to assure proper <br />drainage. <br />5. Upon request and prior to commencement of the contract, the owner of the project will be <br />provided with a list of emergency phone numbers for contact outside of normal business hours. <br />6. Due care will be exercised in protecting from damage all existing facilities, structures and utilities, <br />both above and below ground. <br />7. All employees shall wear or have some method of employee identification such as company <br />shirts or hats at all times while on the owner's property. <br />8. Proof of current conformance to the owner's insurance requirements shall be provided. <br />9. The owner shall be notified of any badly damaged plant material or equipment. <br />10. All equipment and/or plant material damaged as a result of DMS Landscape Services negligence <br />wilt be repaired or replace promptly at no cost to the owner. <br />11. All damage to equipment and/or plant material not resulting from DMS Landscape Services shall <br />reported promptly to the owner with an estimate of costs for correction (if required). <br />12. Repairs due to normal wear will be charged at a cost plus 15% basis for material only. <br />13. Repairs due to vandalism or acts of nature, which DMS Landscape Services has no control over, <br />will be charged at cost plus 15% plus labor at the rates specified herein. <br />14. Additional labor, if required, for repairs or extra work will be charged at the following rates: <br />- Irrigator $38.00 Per Hour • Laborer $25.00 Per Hour <br />15. Twelve general inspections with Owner/Manager Representative, DMS Representative or Area <br />Supervisor per year will be conducted. <br />Initial <br />Initial <br />25C-80 <br />
