EXHIBIT A <br />SCOPE OF SERVICES <br />CONSULTANT shall, at the written request of the Executive Director of Personnel Services, provide <br />ergonomic assessments to help identify and mitigate repetitive motion injuries to City employees. Such <br />tasks include: <br />Preventative Ergonomic Evaluations <br />The objective of these services is to reduce employee exposures to Cumulative Trauma Disorders (CTDs) <br />and Repetitive Motion Injuries (RMIs) through ergonomic evaluations. The targeted groups for these <br />services are employees located throughout the City of Santa Ana. <br />On -site Ergonomic EvulMU6011S to include the following: <br />• On -site employee interview and observation of ergonomic stressors at identified workstations. <br />• Identification of repetitive and forceful motions, awkward postures, environmental factors and <br />employee discomfort that may lead to CTDs or RMIs. <br />• Identification of risk factors that may contribute to the onset of CTDs or RMIs. <br />• Photographs of workstation. <br />• Written office ergonomics report incorporating information on observations and findings, <br />including product recommendations, adjustments and suggestions for improving workstation <br />efficiency and safety. <br />Compensation: 5225.00 per evalm.tion <br />Workers' Compensation /Return -to -Work Ergonomic Evaluations <br />These evaluations are targeted to individuals who have experienced injuries and have specific work <br />restrictions that need to be addressed or those who are returning to work form a non - occupational <br />injury/illness. <br />On -site Ergonomic Evaluutioivs to include the foilowing. <br />• On -site employee interview and observation of ergonomic stressors at identified workstations. <br />• Identification of repetitive and forceful motions, awkward postures, environmental factors and <br />employee discomfort that may lead to CTDs or RMIs. <br />• identification of risl: factors chat may contribute to the Onset of CTDs or RMIs. <br />• Photographs of work environments. <br />• Written ergonomics report :rcorporating information on medical history and work restrictions, <br />observations and findings, including product recommendations, adjustments and suggestions for <br />improving workstation efficiency and safety. <br />Compensation: 5285.00 per cvaluatior. <br />All evaluations mat, he completed rr)1- waled an. a:ron- seated nor,ctutions. <br />
