^ HIV/AIDS Housing -Fifty-five households living with HIV/AIDS were assisted with tenant <br />based rental assistance funded with HOPWA resources. HOPWA funds were also utilized to <br />assist 131 individuals with transitional housing, 16 individuals with emergency housing <br />assistance, 123 with homelessness prevention assistance, and 517 with various supportive <br />services (including housing coordination, life skills and detox services). <br />2. SUMMARY OF ACTIONS TAKEN DURING THE PROGRAM TO DEVELOP A COC STRATEGY: <br />Specific actions undertaken during the report period to assist the homeless through a system of <br />care included the following: <br />a. Actions taken to prevent homelessness -Santa Ana provided nonprofit homeless service <br />providers with ESG funds during the report period in order to help prevent 231 Santa Ana <br />residents from becoming homelessness. On a countywide basis, HOPWA funds were <br />utilized to assist 123 persons with HIV/AIDS from becoming homelessness. <br />b. Actions taken to address emerpency shelter -CDBG and ESG funds were utilized to provide <br />emergency shelter and supportive services for the City's homeless. A total of 1,895 <br />individuals were assisted during the report period with ESG funds. Grant funds were used <br />to offset the operating costs for emergency shelter programs and to provide essential <br />services for the homeless. On a countywide basis, HOPWA funds were utilized to assist 16 <br />persons with HIV/AIDS with emergency shelter and supportive services (Residential <br />Substance Treatment). <br />c. Actions taken to address transitional housing: HUD funding was used to provide transitional <br />housing opportunities for 63 Santa Ana residents.' Regionally, 131 individuals were assisted <br />with HOPWA-funded transitional housing and support services during the 2008-2009 report <br />period. <br />d. Actions taken to transition homeless into permanent housinp: Providers of transitional <br />housing are the key to preparing the homeless for self-sufficiency and movement into <br />permanent housing. During the report period no CDBG or HOME funds were utilized to <br />specifically provide permanent housing for the homeless; however, the SAHA allocated ten <br />rental assistance vouchers to assist transitional shelter graduates attain permanent housing. <br />On a regional basis, HOPWA resources were utilized to assist 55 households access and <br />maintain permanent housing with tenant based rental assistance. <br />The referenced CDBG and ESG accomplishments take into account an overlap of 35 individuals served with both <br />funding sources -Wise Place received CDBG and ESG funding for the same program and reported the same annual <br />accomplishments for both grants. <br />23 <br />Draft 8/26/2009 <br />19F-29 <br />
