Actions taken to address the needs ofsub-populations, e. p. those living on the streets, and <br />new funds such as CoC Super NOFA: Several homeless programs supported by the City of <br />Santa Ana have an outreach component for special needs homeless sub-populations and <br />the chronically homeless. These programs provide access to resources by referral and/or <br />direct contact. These programs include: <br />^ Women's Transitional Living Center, Interval House, Human Options and Laura's House - <br />Program efforts on victims of domestic violence. <br />^ WISE Place -Served on the needs of homeless elderly and young emancipated women. <br />^ Mental Health Association of Orange County -worked with the City's chronic homeless <br />population, providing services and access to housing from a number of "street" <br />locations throughout the City. <br />• Mercy House staffed the Winter Armory Shelter program, which provided night-time <br />shelter, meals, and referral services to individuals living on streets, parks, cars, etc. <br />^ Orange County Health Care Agency - operated a proactive outreach program to assist <br />homeless persons with HIV/AIDS living on the streets. A portion of this program was <br />funded with HOPWA. <br />^ HOPWA Tenant Based Rental Assistance -Funding to assist individuals living with <br />HIV/AIDS maintain independent living situations~'° <br />Santa Ana did not apply for Continuum of Care Super NOFA funds in 2008 or 2009. (Unlike past <br />years, no requests for Certification of Consistency with the City's Consolidated Plan was <br />received by the City by NOFA applicants.) The City of Santa Ana also participated in the <br />development of the regional Continuum of Care strategy by providing valuable mainstream <br />fund information and assigning key staff to participate in the Orange County Continuum of Care <br />Community Forum. The Community Forum was responsible for identifying the gaps and <br />priority needs for the County's 2008 and 2009 associated application for Super NOFA homeless <br />funds. <br />24 Draft 8/26/2009 <br />19F-30 <br />
