City of Santa ulna <br />Scope of Work <br />Name of Organization Interval House <br />Name of Funded Program Domestic Violence Shelter Services <br />Annual Accomplishment Goal — -- <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />220 Persons <br />ll. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract period. <br />C_� Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The expanded Interval House Domestic Violence Shelter Program will provide assistance to 81 Santa Ana victims <br />of domestic violence and their children through emergency and transitional shelter services. <br />Services to be Provided: <br />Emergency Shelter: estimated 60 clients - Emergency shelter programs will offer a safe, caring, and homelike <br />environment for women, adolescents, and children in life -threatening situations and left homeless as a result of <br />domestic violence and abuse. The typical length of stay in emergency shelter is 30-45 days. <br />Transitional Housing: estimated 21 clients - Since 1982, Interval House has maintained additional homes and <br />programs for those families needing a longer transition period to become self-sufficient, contributing members of <br />their communities. The typical length of stay at our second -stage homes varies from 60 days to 24 months. <br />Individual and Group Counseling: Sensitive and flexible counseling services will be available for adults, teens, <br />children, and elders in shelter and outreach programs. Counseling services range from peer support to therapeutic <br />individual and group counseling provided by skilled professional and para-professional counselors in over 45 <br />different languages. <br />Other Support Services available will include legal assistance, health and wellness services, social service <br />advocacy, youth violence prevention programs, multi -cultural programs, and other support services needed by <br />victims. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
