City of Santa Ana <br />Performance Outcome Tracking <br />Activity/ <br />Program <br />Outcome Tracking <br />Goal <br />HUD Outcome Tracking <br />HUD Goal <br />Youth Services <br />Track children/youth seeking <br />90% of childrentyouth <br />Number of persons assisted: <br />Accessibility for the <br />referrals that were linked to <br />seeking referrals will be <br />-with new access to a service <br />purpose of creating <br />the services sought <br />linked to the services <br />-with improved access to a service <br />Suitable Living <br />sought. <br />-where activity was used to meet a <br />Environments <br />quality standard or measurably improved <br />quality, report the number that no longer <br />only have access to substandard service <br />Describe how you will gather the tracking information, noted above, for that activity. <br />The following are methods that will be used to track the City and HUD tracking requirements. <br />1) Track improvement in pre -survey, post -survey. <br />Improvement can be documented by administering pre -survey as part of the registration and then <br />administering a post -survey 6 months later, after having received at least five tutoring sessions. <br />2) For education -based programs, improve pre -test post-test scores by 50%. <br />The scores students receive from the pre- and post -survey will be used to determine whether they have <br />improved their scores by 50% or not. <br />3) Number of persons assisted: <br />a) With new access to a service: <br />The library's Learning Center Database (LCD), which can provide new registrations and attendance per <br />month. <br />b) With improved access to a service: <br />The library's LCD tracks the number of tutoring sessions. <br />c) Where activity was used to meet a quality standard or measureably improved quality, report the <br />number that no longer only have access to substandard service: <br />The library's LCD tracks the number of unique users. <br />Exhibit A-1 <br />Page 1 of 1 <br />
