The database will track the participant involvement in all program <br />activities. All activities will include sign -in sheets to track participation and time sheets will be used for <br />participants to track the time they volunteer at the recreation centers, city-wide events, and community <br />projects. The time sheets will be gathered each month to keep track of the number of hours completed by <br />the family members throughout the program year. <br />All activities are evaluated by measuring the number of participants served, type of participant (gender, <br />age, ethnicity, race, income level, female head of household), level of participation, number of volunteers, <br />number of volunteer hours completed per family and per teens, type of volunteer service in the City's <br />recreation centers and City sponsored community events and/or programs. The documentation that will <br />be used for tracking; Registration Forms, Volunteer Applications, Volunteer Monthly Time Sheets, <br />Attendance Sheets (volunteers, community service, Family activities, Family PRIDE Club meetings), and <br />a survey to track participants to determine if they were linked to services sought. <br />Data will be entered in the data base for each family and/or program participant and quarterly reports will <br />be gathered to report on family/teen recreation and volunteer activities. PRCSA will use a variety of <br />tracking tools to gather data to track progress in a number of areas. The experience of PRCSA in this <br />area will facilitate the development and implementation of measurement tools and a thorough analysis of <br />the impact of the program on participants' behavior and lifestyle changes. <br />Exhibit A-1 <br />Page 1 of 1 <br />
