City of Santa Ana <br />Scope of Work <br />Name of Organization SOUTHWEST MINORITY ECONOMIC DEVELOPMENT ASSOCIATION (SMEDA) <br />Name of Funded Program EVICTION PREVENTION <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />4591 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract period. <br />Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />SMEDA will use the funding for this program to assist Santa Ana residents access our emergency rental and <br />utility assistance programs to avoid eviction and utility disconnection. During the initial intake interview, SMEDA <br />staff will ascertain whether the client has any other emergency critical needs and if we can resolve those needs <br />through our services or by referrals. <br />Many of the other services that we have, such as food, clothing, diapers, hygiene and similar items, will offset <br />our clients' direct expenses. They also help them cope with their financial crisises, and ultimately allow them to <br />use their already limited incomes toward other expenses or forego similar crises the following month. We will <br />also advise clients that their receipt of assistance will allow us to track their status from six to twelve months <br />thereafter. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
