Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />F----2-0-001 Persons <br />II. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />the primary goals fof hte outreach program is to first, locatae, then engage the mentally -ill homeless of Santa <br />Ana and assess their social, mental and medical needs. Secondly, program staff will assist consumers in <br />accessing the mental health, social services and medical services that they need to improve their situations and <br />end their homelessness. An important by tertieary goal is to provide mental health expertise and networking <br />to services offerented by existing general homeless services providers in Santa Ana. The Outreach team will <br />egage 550 unduplicated homeless individuals in the City of Santa Ana over the course of the program year and <br />provide at least 3000 units of services to those involved in the program. Of the 550 unduplicated homeless <br />individuals engaged in the City of Santa Ana, 300 will be actively linked to the psychosocial, mental health and <br />medical services that they need to begin the process of ending their homelessness and provide them a more <br />successful and satisfying life. Through thiis grant, MHA will provide a half-time bilingual/bicultural spanish- <br />speaking Outreach Worker to the various existing sites providing services to the general homeless in Santa Ana <br />to assess for signs of mental illness and to act as a port of entry to obtaining mental health services. Homeless <br />Outreach will be provided by this bilingual outreach counselor exclusivelyto the City of Santa Ana and will <br />include street outhreach and ride-alongs with the Santa Ana Police, in addition to outreach to local homeless <br />providers, feeding centers and shelters of Santa Ana. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter.
