All information is <br />input into a central Database (located in the Program office at 2416 S. Main Street, Santa Ana) where <br />statistical information is extracted as needed. Program Staff meet weekly to discuss program <br />participants, while processing individual service plans and to monitor ancillary services at a variety of <br />Santa Ana sites each week where homeless individuals gather for services. <br />The quantities outcome measurement will include all data pertaining to all areas of access client as <br />received from outreach program staff. These include, but are not limited to all pertinent reports to <br />programs as well as administrations to formulate other ways to help and guide clients to end <br />homelessness. The collection of data enables MHA to guide the staff in hand to get areas of access <br />clients we would not normally find. <br />Due to the sensitvitiy of being homeless and with a persistant mental illness accomplishments are <br />achieved, yet the timeline could have a number of variances. Although clients are reluctant to access <br />services with a sandwich or a bottle of water or even a friendly face seen time and time again clients can <br />become more comfortable and more willing to access services offered therefore starting the process to <br />end homelessness. <br />Exhibit A-1 <br />Page 1 of 1 <br />
