The followuig program elements will be made available to eligible residents expected to <br /> enroll in the FEZ Daisy Wheel Network Program for participation in activities aimed at <br /> providing a comprehensive strategy that addresses employment and training needs: <br /> i. employment strategies/job preparation <br /> ii. education (English Language and basic skills) <br /> iii. supportive services <br /> C. Performance Outcomes E~iiploy~lrent Se~•vices <br /> a. 80 will be served <br /> b. Of the 80 FEZ residents sewed, 80 (100%) will receive pre-employme~it services, <br /> including workforce preparation workshops, career counseling, Job development, job <br /> referrals, and labor market information; <br /> c. Of the 80 FEZ residents served, 24 (30%) will be placed in unsubsidized employment; <br /> d. Of the 80 FEZ residents served, 80 (100%) will Ue provided additional services as <br /> needed, including vocational ESL classes; <br /> e. Planned eruolhllents by month: <br /> March A u•il 117a June. <br /> 20 20 20 20 <br /> <br /> VII. THE CAMBODIAN FAMILY/TCCDC PROGRAMMATIC RESPONSIBILITIES <br /> EMPLOYMENT SERVICES` <br /> 1. The rivo partner agencies will provide eligible FEZ residents with services designed to meet <br /> the FEZ Board requirements. A participant will be provided with one or more of the <br /> following services to assist them in obtaining unsubsidized employment and self-sufficiency: <br /> • Case management <br /> • Comprehensive support services <br /> • Job development <br /> • Placement and retention services <br /> • Job training /education <br /> • Workforce preparation workshops <br /> 2. Evaluate participants for eligibility and assign those eligible for program services. <br /> 3. Perform client assessments, including assessment of English language ability. <br /> 4. Maintain records of clients activities and job placement. <br /> 5. Attend partner meetings. <br /> 6. Provide quarterly progress reports. <br /> 7. Provide necessary infoi7nation far conducting informal and formal monitoring of program <br /> progress. <br /> 8. Meet performance measures. <br /> 9. Submit Monthly Statistics Report documentation to The Cambodian Family staff by the Sth <br /> of every month. Report should include new eruollments and a continuous Activity <br /> Eruollment Form. <br /> 10. Maintain a current and complete file (electronic as well as a hard copy) for each FEZ <br /> participant: eligibility documentation; registration form; enrollment form; assessment; <br /> Individual Service Plan (ISP}; EEO/Grievance; case notes; supportive services <br /> docmnentation; attendance records; training documentation; etc. <br /> EXHIBIT A-2 <br /> Page 2 <br /> Zoc-9 j <br /> <br />
