<br />The Cambodian Family <br />FEZ Community Empowerment Program <br />Scope of Work <br /> <br />I. TERM <br />March 1, 2010 through June 30, 2010 <br /> <br />II. PURPOSE <br />The FEZ Community Empowerment Program will continue to provide vocationaI/occupational <br />training, employment strategies, education, and supportive services to individuals who reside in the <br />Federal Empowerment Zone. <br /> <br />III. POPULATION TO BE SERVED <br />The population to be served are Federal Empowerment Zone residents 18 years and older. <br />Eligibility will be determined by age, address, and by Right to Work documentation. <br /> <br />IV. SERVICES WILL BE PROVIDED AT: <br />The Cambodian Family <br />1111 E. THE CAMBODIAN FAMILY ADMINISTRATIVE RESPONSffilLITIES <br />1. Develop contracts for partners <br />2. Provide orientation and training on eligibility, referral process, assessment, joint policies and <br />procedures, fiscal billing, documentation, etc. <br />3. Develop joint forms such as application, screening/intake tool, ISP, referral form, <br />4. Perform Program Reporting <br />5. Provide partners with on-going technical assistance <br />6. Facilitate collaborative meetings <br /> <br />VI. PROGRAM DESIGN, ELEMENTS, AND ADDITIONAL OUTCOMES FOR THE FEZ- <br />COMMUNITY EMPOWERMENT PROGRAM - EMPLOYMENT SERVICES <br />A. Program Design: <br />a. Provide an intake assessment (for screening) of each FEZ resident that includes a review <br />of academic and occupational skills levels as well as service needs; <br />b.
