iv. Class Fees <br /> i. Class pricing is dependent on session length per season and subject <br /> to City staff approval. Class pricing is limited to no more than a 5% <br /> increase annually. <br /> ii, Each participant shall pay class registration fees as established by <br /> City. Provider may not waive class participation/registration fees. <br /> Only registered participants paid in full may participate in class. <br /> iii. Any refunds to participants will be made in accordance with City <br /> policy. <br /> B. Provider Responsibilities: <br /> i. Planning and delivering engaging and age-appropriate guitar classes as <br /> defined by class curriculum and description to be approved by City staff. <br /> ii. Ensuring the safety and well-being of all participants during class sessions. <br /> ■ Provider will immediately report to City staff, by phone or email, any <br /> injuries as a result of class participation, damages to the facility that <br /> could cause potential injury to a class participant and/ or require <br /> facility repairs. <br /> ■ Provider will notify parent/ guardian of minors under the age of 18 <br /> and city staff regarding any injuries experienced during class. <br /> iii. Submitting seasonal program proposals to City staff for seasonal approval <br /> of class descriptions, details, and schedules, Proposals must be submitted <br /> in writing by way of the provided City form(s) to City staff for review and <br /> approval at least 60 days prior to the start of the new season, unless <br /> otherwise specified by City staff. City staff will review and approve written <br /> proposals based on community needs, facility availability, and alignment <br /> with City goals. <br /> iv. Adhering to all City policies and procedures related to the use of facilities <br /> and interaction with participants. <br /> v. Adhering to all City deadlines for required documentation. <br /> vi. The Provider's organization is responsible for fingerprinting, monitoring, and <br /> managing all staff that will be instructing. <br /> Page 12 of 13 <br />
