City of Santa Ana <br /> 23-HHAP-10015 <br /> Page 9 of 24 <br /> Homeless Housing, Assistance, and Prevention Program Round 4 (HHAP-4) <br /> Standard Agreement <br /> EXHIBIT B <br /> BUDGET DETAIL and DISBURSEMENT PROVISIONS <br /> 1) Budmet Detail & Changes <br /> The Grantee agrees that HHAP-4 funds shall be expended on uses that support <br /> regional coordination and expand or develop local capacity to address immediate <br /> homelessness challenges. Such activities must be informed by a best-practices <br /> framework focused on moving people experiencing homelessness into permanent <br /> housing and supporting the efforts of those Individuals and familfes to maintain their <br /> permanent housing. <br /> The Grantee shall expend the HHAP-4 funds on eligible activities as detailed in <br /> Health and Safety Code section 50218.7(e) and section 50220.8, subdivisions (e), <br /> and (f). <br /> 2) General Conditions Prior to Disbursement <br /> All Grantees must submit the following forms prior to HHAP-4 funds being released: <br /> • Request for Funds Form ("RFF") <br /> • STD 213 Standard Agreement form and initialed Exhibits A through F <br /> STD 204 Payee Data Record or Government Agency Taxpayer ID Form <br /> 3) Disbursement of Funds <br /> Initial Disbursement <br /> Fifty percent of a grantee's HHAP-4 funds will be disbursed to the Grantee upon <br /> receipt, review and approval of the completed Standard Agreement and RFF by <br /> Cal ICH, the Department of General Services (DOS) and the State Controller's <br /> Office (SCO), <br /> The RFF must include the proposed eligible uses and the amount of funds proposed <br /> for expenditure under each eligible use. The Initial disbursement of HHAP-4 funds <br /> will be disbursed in one allocation via mailed check once the RFF has been received <br /> by the SCO.
