~tandUp ~ PCT' kids <br />ORANGE COUNTY <br />The case manager will conduct an initial assessment to determine the participant's need for <br />supportive services. This assessment will be reviewed regularly and updated as appropriate. All <br />services provided will be documented through case notes , and a log of issued services will be <br />maintained for tracking and accountability purposes. <br />Types of Supportive Services <br />Supportive services may include, but are not limited to , the following: <br />• Transportation-Bus passes or gas cards costs to attend training or <br />employment activities. Service is limited to once per month. No replacement <br />bus pass or gas card will be issued in the event the participant loses his/her bus <br />pass or gas card. <br />• Clothing and Work Attire-Purchase of uniforms, protective gear, or <br />professional clothing needed for job interviews or work. <br />• Housing and Utilities -Temporary assistance with rent or utilities to prevent <br />disruption to training or employment. Housing assistance is limited to one-time <br />support, with the possibility of more support upon request based on funding and <br />availability. <br />• Technology and Supplies -Laptops, internet access, or training materials <br />necessary to complete WIOA-authorized programs. <br />• Health and Safety -Emergency assistance related to health, safety, or other <br />personal emergencies impacting participation. <br />Process <br />1. Request Submission <br />a. Participants must complete a Supportive Services Request Form <br />detailing the type of support needed and the reason for the request. <br />b. Participants must provide documentation of efforts to secure assistance from <br />other sources before submitting the request. <br />c. The request form must be submitted to the participant's case <br />manager or designated WIOA staff. <br />2. Review and Approval <br />a.
