Work Experience Agreement and Monitoring <br /> • The participant, employer, and case manager must sign a Work Experience Agreement <br /> outlining: <br /> o Start and end dates. <br /> o Expected work hours and schedule. <br /> a Performance expectations acid evaluation criteria. <br /> o Wage and benefits information(if applicable). <br /> • Monitoring and evaluation should include: <br /> a Regular check-ins between the participant and case manager. <br /> o Feedback from the employer on performance and skill development. <br /> o Adjustments to the training plan based on participant progress. <br /> 5. Incentives and Payments <br /> • Participants may be eligible for incentives based on performance and achievement of specific <br /> goals tied to the work experience. <br /> • Any incentive payment must be documented and approved by the case manager and program <br /> supervisor. <br /> • Paid work experiences must comply with minimum wage laws and labor standards. <br /> 6. Work Experience Duration and Limits <br /> Local policy must define the following: <br /> • Duration—Length of time a participant may be in a work experience. <br /> • Hours per week—Number of hours a participant is allowed to work each week. <br /> • incentives and stipends—Maximum allowed payment amounts and conditions for issuance. <br /> 7. Supportive Services for Work Experience <br /> Participants may receive supportive services to ensure successful participation in work experience, <br /> including: <br /> • Transportation assistance(bus passes, gas cards). <br /> • Childcare support. <br /> • Work attire or uniforms. <br /> • Tools or equipment required for the job. <br /> • Technology access(Wi-Fi, laptops,etc.). <br /> Participants must submit a Supportive Services Request Form and document their efforts to obtain <br /> assistance from other sources before receiving supportive services through W10A funds. <br /> EXHIBIT D <br />
