Dear City Leadership, I am submitting the following request for administrative review regarding Case CP25-02191 and Citation AC 252521a for 2601 N. Grand Ave, Apt 229 (Fairhaven Apartments). The letter below explains multiple inconsistencies and concerns about the integrity of the record. Please confirm receipt and advise on next steps for review. I am requesting an administrative review of Code Enforcement Case CP25-02191 and Citation AC 252521a for Fairhaven Apartments (1300 Fairhaven Ave / 2601 N. Grand Ave, Apt 229). Specific problems: • Two pages of the citation are hand -marked "6/21/25 VOID," yet the City database logs multiple violations and inspections. • The Addendum lists only SAMC 41-1304(c) (lighting shields), while the City's own case description also lists "ceiling separation and structural damage." Those items are omitted from the citation packet. • The case shows "Date Closed: 06/02/2025," which is before the listed compliance date of 06/16/2025. • On 05/29/2025 the officer states an administrative citation for exterior lighting was issued; on 05/30/2025 (night) another note states there is "no indication of excessive/hazardous lighting." There is no written reconciliation explaining the change. • The packet contains duplicate forms, blank signature lines, and standard "pay/appeal" language even though no fine was assessed —suggesting the packet was never properly completed. Condition of the record: The 11-page packet I received is physically torn and folded in several corners and appears incomplete, with duplicate boilerplate pages. The handwritten "VOID" markings raise further concern that the record was not preserved or maintained with the integrity required for an official enforcement action. Requested actions: 1. A written explanation of why the citation was marked "VOID," who approved that action, and why the case was closed before the compliance deadline. 2. Confirmation whether the lighting violation remains a violation, whether shielding was required/installed, and the results of any follow-up night inspections.
