excess of the amount of the City contribution to the Cafeteria Benefit Plan specified <br /> above shall be borne entirely by the employee. <br /> F. Cafeteria Benefit Plan. The City provides a Cafeteria Benefit Plan adopted in <br /> accordance with the provisions of Internal Revenue Code § 125 for employees <br /> covered by this MOU. <br /> 1. Employees will be eligible to receive through the Cafeteria Benefit Plan, <br /> the cash difference (subject to taxation as wages) if the monthly City <br /> allowance exceeds the monthly cost of the Medical and Dental monthly plan <br /> premium cost. <br /> 2. For each such employee who is covered under a group sponsored health <br /> plan, the City will pay the employee a cash-in-lieu payment (subject to <br /> taxation as wages) each month in an amount equal to the City contribution <br /> for the"employee-only" coverage for medical. The above specified amount <br /> of City contribution towards such coverage waived will be applied to earned <br /> income. If said employee waives, in writing, City-paid coverage, said <br /> employee shall provide proof of medical insurance coverage in a group- <br /> sponsored health plan. Said waiver shall include a provision warning such <br /> employee that re-entry into any of the City-sponsored plans is allowed only <br /> at open enrollment,unless there is a qualifying event, and will require proof <br /> of insurability for such employee and/or eligible dependents. <br /> 11.2 Dental Insurance. The City agrees to contribute towards the Cafeteria Benefit plan the <br /> payment of premiums for dental insurance plans provided by the City for employees <br /> covered by this MOU and their eligible dependents on the following basis. <br /> A. The City shall contribute an allowance towards the employee's Cafeteria Benefit <br /> plan for dental insurance premiums for eligible employees and their qualified <br /> dependents in the amount of one hundred thirty dollars ($130) per month per <br /> employee. <br /> B. Any contribution necessary to maintain benefits under said dental plans in excess <br /> of the amount of the City contribution to the cafeteria plan specified above shall be <br /> borne entirely by the employee. <br /> C. For each such employee who is covered under a spouse's non-City sponsored dental <br /> plan, the City will pay the employee a cash-in-lieu payment(subject to taxation as <br /> wages)each month in an amount equal to one-hundred percent(100%)of the City's <br /> contribution, if the employee waives, in writing, City-paid coverage. Such waiver <br /> shall include a provision warning such employee that reentry into any City- <br /> sponsored plan is allowed only at open enrollment, unless there is a qualifying <br />
