B. With respect to employees who do have dependents included under <br /> the program, the City will contribute toward the payment of premiums for <br /> health insurance on the following basis: <br /> (1) For employees who have less than one year of full-time <br /> service, an amount equal to 100% of the premium for "employee-only" coverage <br /> but no amount for dependent coverage. Such employees shall have the option to <br /> purchase dependent coverage under the plans provided by the City for its <br /> employees by personally paying the amount of difference between "employee-only" <br /> coverage and "family plan" coverage. <br /> (2) For employees who have completed one year of full-time <br /> service; an amount equal to 90% of the total premium applicable to the "family <br /> plan" coverage. <br /> (3) Notwithstanding the foregoing, with respect to "lateral <br /> transfer" police officers and employees reappointed to the service of the City <br /> in accordance with Section 9-114 of the Civil Service Rules and Regulations, tht <br /> City will contribute an amount equal to 90% of the total premium applicable to <br /> the "family plan" coverage as soon as such employees are eligible for such <br /> coverage after the date of their appointment/reappointment. <br /> Additionally, the City will apply said 90% premium payment benefit, retroactive <br /> to July 1, 1981 , to any such lateral transferee or reappointee appointed/ <br /> reappointed within the two-year period preceding the effective date of this <br /> Agreement who had family plan coverage on July 1, 1981 . <br /> Section 2 . Dental Insurance. The City agrees to contribute <br /> the following amounts toward the payment of premiums for dental <br /> insurance plans provided by the City for employees covered by <br /> this Agreement and their eligible dependents ; <br /> A. With respect to sworn , "safety" police personnel , <br /> up to a maximum of fifteen dollars and thirty-two cents ($15 . 32 ) <br /> per month per employee. <br /> B. With respect to "miscellaneous" employees, up to a <br /> maximum of twelve dollars ( $12. 00) per month per employee . <br /> C. Any contribution necessary to maintain benefits <br /> under said dental plans in excess of the amounts set forth above <br /> shall be borne entirely by the employee. <br /> Section 3 . Long-Term Disability (L.T.D. ) Insurance. The <br /> City shall continue to contribute the following amounts toward <br /> the payment of premiums for employees covered by this agreement <br /> under the existing long-term disability plan maintained by the <br /> City for its employees; <br /> A.
