_____________________________________________________________________________________________________________________ <br />CONFIDENTIAL ASSOCIATION OF SANTA ANA (CASA) MOU: JULY 1, 2025 THROUGH JUNE 30, 2028 <br />Page 45 <br />13139922.1 SA370-001 <br />Effective January 1, 2026 the City shall pay one hundred percent (100%) of the premium <br />cost for a sixty (60) day elimination period with a maximum benefit of ten thousand dollars <br />($10,000) per month for a long-term disability insurance plan for employees. <br /> Life Insurance. The City shall maintain in effect its existing life insurance plans covering <br />employees, including term life insurance coverage for each affected employee in the <br />amount of fifty thousand dollars ($50,000) plus fifty thousand dollars ($50,000) Accidental <br />Death and Dismemberment (AD&D) coverage at no cost to the employee. <br /> The City shall retain the right to change health, dental, and life insurance carriers, <br />administer the insurance benefits provided thereunder, and select and/or change any excess <br />or supplemental insurance carriers as a part of any self-insurance plan during the term of <br />this MOU, provided that employees continue to receive equivalent benefits and provided <br />that the parties have met and conferred before the benefits changes are made. <br /> When there is a need to discuss matters relating to employee insurance and the City <br />believes it would be beneficial to involve an Insurance Committee, CASA shall have an <br />equal number of representatives as the City on such a committee, and the Committee shall <br />meet as necessary. <br /> Retirement Health Savings Plan. Employees participate in the City’s Retirement Health <br />Savings Plan (RHS) subject to the following: <br />A. Employees shall contribute one and one-half percent (1.5%) of their base pay each <br />pay period to the RHS plan, which amount shall be deposited into the employee’s <br />individual RHS account. This contribution shall be made on a pre-tax basis. <br />B. The City will contribute one percent (1%) of the employee’s base pay deposited <br />into their individual RHS account each pay period. <br /> <br />12.5 <br />12 .6 <br />12.7 <br />12.8
