However,an employee may be granted up to an additional two (2) <br /> months of leave, at the discretion of the City Manager with recommendation from the <br /> Executive Director of Human Resources, for a total of up to six (6) months in which to <br /> recover from the disability. <br /> As with all other temporary disabilities, a physician's certificate is required to verify the <br /> extent and duration of the temporary disability. <br /> An employee who plans to take pregnancy leave must give reasonable notice of at least <br /> four (4) weeks before the commencement of the leave and include the estimated duration <br /> of the leave. <br /> The City will continue to contribute towards the employee's health insurance coverage in <br /> accordance with state and federal legislation. The employee will be required to pay a cash <br /> premium to continue coverage while on a leave of absence without pay. <br /> 10.10 Catastrophic Leave <br /> In order to assist employees otherwise granted a leave of absence without pay by the City <br /> Manager due to a catastrophic, non-industrial medical condition or injury, the City shall <br /> maintain a Catastrophic Leave Donation Program. <br /> Nothing in this section shall be construed to alter City policies and procedures as provided <br /> in the Charter or ordinances of the City or other provisions of this MOU with regard to <br /> granting unpaid leaves of absence. <br /> For the purpose of this program, a "catastrophic" condition shall mean any significant <br /> personal tragedy such as a life-threatening illness or severe nonindustrial illness or injury <br /> lasting more than two (2) weeks, which requires the employee to need personal time off <br /> beyond the amount of leave time they have accrued. Maternity leave or elective surgery, <br /> absent significant unplanned complications preventing the employee's return to work,are <br /> not considered catastrophic. <br /> The Catastrophic Leave Donation Program shall cover the uncompensated time prior to the <br /> employee's becoming eligible for the LTD benefits. <br /> A. Guidelines <br /> It shall be understood that all donations under this procedure are voluntary and <br /> subject to taxation for the recipient. <br /> 1. Employees may donate vacation, compensatory time, or one (1) floating <br /> holiday to the eligible employee. In no event shall sick leave be donated. <br /> 2. Employees shall be provided a two (2) week period to submit donations. <br /> Donations received after this two (2) week period shall not be processed.
