9 HISTORICALLY UNDERUTILIZED BUSINESS (HUB) CERTIFICATION <br />3 <br />Businesses that have been certified by the Texas Building and Procurement Commission (TBPC) or other qualified <br />agency as Historically Underutilized Business (HUB) entities are encouraged to indicate their HUB status when <br />responding to this proposal invitation. The electronic catalogs will indicate HUB certifications for vendors that <br />properly indicate and document their HUB certification on this form. <br />Select one of the available options: <br />OPTION A: My business has NOT been certified as HUB. <br />OPTION B: I certify that my business has been certified as a Historically Underutilized Business (HUB), and I <br />have/will upload the certification information into the "Response Attachments" Tab located in this online bidding <br />event. <br />OPTION A - Not HUB <br />9 DISCLOSURE OF INTERESTED PARTIES <br />4 <br />Texas state law requires the Disclosure of Interested Parties be filed with a public entity, including regional service <br />centers and school districts, for any contract which: <br />(1) requires an action or vote by the governing body; or <br />(2) has a value of $1 million or more; or <br />305. <br />NOTE: This form is not required if the vendor is a publicly-traded business entity, including a wholly-owned <br />subsidiary of the business entity (a company in which ownership is dispersed among the general public via shares <br />of stock which are traded via at least one stock exchange or over-the-counter market). <br />If you are required by law to submit this form, it must be completed online at the Texas Ethics Commission website. <br />Obtain a numbered certificate and click the link below to access the instructions and to complete this required form. <br />Upon completion, vendors required to submit the form must attach it to the proposal via the "Response <br />Attachments" Tab. <br />Click here to complete the form on the Texas Ethic Commission's 1295 Form webpage. <br />Please note: The District must verify receipt of all required 1295 forms received within 30 days on the Texas Ethics <br />Commission website. This verification does not indicate a contract award. Contract awards will be issued via direct <br />communication from the AISD Purchasing Department.
