On 11/03/20, C.O. Fernandez began her shift at 1500 hours and was assigned to processing of new <br />inmates.At approximately 1800 hours, she went to her regular assigned duties, monitoring MOD 3. <br />MOD 3 consists of two separate wings, MOD C and MOD D.At approximately 2130 hours, she took over <br />both MODULES. <br />During her shift, she performs two welfare checks every hour.She explained her welfare checks are not <br />QHFHVVDULO\HYHU\òKRXUEXWDWWKHYHU\OHDVWWZRHYHU\KRXUShe usually starts her welfare checks on the <br />bottom floor and works her way up to the second floor.Between 2130-0000 hours, she performed 3-4 <br />welfare checks.At approximately 2345 hours, she completed her last welfare check before going on her <br />lunch break.She noted nothing out of the ordinary during her last welfare check or the previous ones. <br />On 11/04/20 at approximately 0005 hours, C.O. Fernandez went on her hour long lunch break. C.O. M. <br />Valenzuela #2748 relieved her.At approximately 0105 hours, she came back from her lunch break.She <br />made it a habit to check the log for the last cell check whenever she comes back from a break.She spoke <br />to C.O. Valenzuela for a few minutes before she left on her lunch break.C.O. Valenzuela had nothing <br />unusual to report and left on her break. <br />C.O. Fernandez signed back on to her workstation and checked the logs to see the last check was done at <br />0104 hours.At approximately 0130 hours, she began her welfare check by starting on the lower tier of 3D. <br />After completing the lower tiers, she went up to the second floor.During her welfare check, she had her <br />flashlight out since it was necessary for her to use it while visually looking through the cell door windows. <br />As she reached cell, # 3D24, she could not remember if the light inside his cell was on.She thought <br />she used her flashlight to look into cell but was not certain.She visually looked into his cell, was not <br />sure of her observations, and took a second look.She saw hanging and immediately radioed for <br />Officers and Medical staff to respond for a ³A.´She saw that the bunk sheets were wrapped around <br /> neck. <br />When Officers arrived, C.O. Fernandez opened cell.Once RN and C.O. J. Elizondo #3106 <br />arrived, she went into the cell and saw that face was pale.When she touched and felt that <br />was still warm, . <br /> <br /> was situated between the bunk and the toilet with his face facing the door.He was in a seated <br />position with his legs out towards the door.His buttocks was not touching the floor <br /> .C.O. Fernandez got behind <br /> . <br /> attempted life saving measures until she <br />was relieved by co-workers. <br />C.O. Fernandez said she had no contact with that night.She had minimal contact with him during his <br />stay and was unaware of his history.He caused her no problems and seemed respectful.That concluded <br />the interview.For further, refer to the audio recording retained by Investigator Toth. <br />F.
