City Jail and entered through the sally port area. Correctional Supervisor C. Villa-Andrade escorted me to <br />Module 3D by where was being housed. With me was Inspector Sergeant M. Macchiaroli #2704 from <br />the Santa Ana Police Department Internal Affairs Unit. <br />05:46 <br />We arrived at Module 3D and checked in with Officer K. Gray #3412 who was maintaining a crime scene <br />personnel log. <br />05:47 <br />Supervising District Attorney Investigator B. Tanner and Sergeant J. Garcia #3030 provided another briefing <br />regarding the circumstances related to the in-custody death. Present were members of the Santa Ana <br />Police Department Homicide Unit, Orange County District Attorney¶s Special Assignments Unit, Orange <br />County Crime Lab, Correctional Officers and Supervisors from the Santa Ana City Jail, Inspectors from the <br />Santa Ana Police Department Internal Affairs Unit, and Patrol Officers form the Santa Ana Police <br />Department. <br />05:55 <br />We were escorted to cell (D24) and viewed its current condition. There was no one inside at this <br />time. I observed two blankets around the top metal bunk. Both were tied around the bunk with one being <br />closer to the foot of the bed. We were informed was located at the foot of the bed between the bunk <br />and the toilet. Refer <br />to Detective Corporal M. Lemmon #31741 and CSI photographs for further details on its condition. <br />06:08 <br />I cleared the scene and responded to Orange County Global Medical Center. <br />06:30 <br />I arrived at Orange County Medical Center and checked in with Officer J. Martinez #3702 who was <br />maintaining a crime scene personnel log within the Emergency Room. Along with him was Officer J. <br />Castellanos #3080. was lying in ³Bed ´We maintained in the Emergency Room until members of <br />the Orange County Crime Lab and Orange County Coroner¶s Officer arrived to process the scene. There <br />were a total of 7 patient beds in the Emergency Room labeled #5-9. <br />06:49 <br />While waiting for the assisting personnel, DA Investigator Sheldon interviewed RN who was <br />the nurse who tended to when he arrived. Also present were Inspector Sergeant Macchiaroli and I. <br />The interview was digitally recorded and retained by DA Investigator Sheldon. For details of the interview, <br />refer to DA Investigator Sheldon¶s report documented under OCDA¶s Special Assignment case number SA <br />20-027. <br /> was the tending physician. His shift had ended and was not present to be interviewed. <br /> confirmed the time of death was at 02:15. <br />07:06 <br />Forensic Specialist H. Peplowski #8975 arrived and took overall photographs of current condition. <br /> was lying on a gurney in a supine position and covered with a medical blanket. <br />07:20 <br />Deputy Coroner A.
