Services <br />Human Options <br />Guidance and <br />will focus on increasing access to safety planning, field -based advocacy, crisis intervention, legal <br />Empowerment — Santa <br />protections, and trauma -informed counseling to improve safety, emotional well-being, and housing stability <br />Ana BRIDGE <br />access to community resources and supports, including shelter access. <br />Lutheran Social <br />Services of <br />Victims Intervention <br />$39,462 <br />The Victims Intervention Project (VIP) will provide women who are victims of crime with evidence -based <br />Southern <br />Program <br />recovery services — direct mental health services, rental assistance, hotel/motel vouchers, intensive case <br />California <br />management, transportation, life skills training and other supportive services. <br />Mary's Shelter <br />Aftercare case managers will help young moms develop a transition living plan to explore transitional housing <br />DBA Mar 's <br />y <br />Expanded Aftercare for <br />$39,462 <br />options and coordinate linkages to public benefits, such as Cal -Fresh, CaIWORKs, WIC, and Medi-Cal. <br />Path <br />Teen Moms <br />Santa Ana residents will receive monthly home visitation and group health education that measurably <br />MOMs Orange <br />Maternal Child Health <br />$39,462 <br />improve the health status of motheres and babies living in poverty. Both parents will be provided with <br />County <br />Coordination Program <br />classes, education, and support that will teach them with coping skills, resiliency, and strategies for self -care <br />to improve their mental health. <br />This program will provide intenstive navigation and empowerment designed to walk families from crisis to <br />MOSAIC <br />Mosaic Lifeline Initiative <br />$39,462 <br />capability. Families will receive one-on-one coaching from bilingual family navigator, create a family action <br />Movement <br />plan and provides on -hands guidance to varios resources. Therapy sessions and workshops will be <br />provided. <br />The program is a community outreach initiative that expands and extends recreational and educational <br />Nati's House <br />$39,462 <br />services to low-income families residing in the City of Santa Ana at night during the summer.
