Montez #325 Approved By: <br />Date of Report: 09/24/2018 Date: 10/01/2018 <br />Page 2 of 2 <br />On July 1, 2018, at 2338 hours, CHAVEZ along with OCFA Engine 71 personnel were <br />dispatched to a shooting on the second floor of the parking structure located at 1020 West Civic <br />Center Drive, Santa Ana. They arrived at 2341 hours. <br />When they arrived, CHAVEZ observed DONADONY BLAKE TAYLOR handcuffed with his <br />hands behind his back and he was lying on the slab lying on his back. Police officers on scene <br />were performing cardio pulmonary resuscitation (CPR). TAYLOR did not have a pulse, was apneic <br />and did not respond to verbal painful stimuli. <br />TAYLOR had a chest seal on mid sternum covering a bullet hole, a bullet hole on the left <br />bicep, and two Taser barbs imbedded on the front of his chest. CHAVEZ had the officers stop CPR <br />and after assessing he still had no pulse and was not breathing. CHAVEZ continued CPR, placed <br />TAYLOR on a Zoll monitor and ventilated TAYLOR with oxygen. Th e Traumatic Cardiac Arrest <br />Protocol was then followed which consisted of: establishing an airway, a rhythm by placing him <br />on a monitor, an intravenous line by doing an intraosseous infusion on the left leg. Due to TAYLOR <br />having Pulseless Electrical Activity (PEA) no shock was advised. They applied a tourniquet to the <br />left bicep to stop the bleeding. They administered a total of four (4) dosages of epinephrine. <br />Due to TAYLOR having diminished air on his right lung they were directed by the base hospital t o <br />perform a needle decompression. <br />At 2358 hours, TAYLOR was placed in a CARE Ambulance and transported to Orange <br />County Global Medical Center – Santa Ana (OCGMC). <br />At 0009 hours, they arrived at OCGMC and transferred TAYLOR’S care to the Emergency <br />Room medical staff. <br />CHAVEZ had no additional information to add. The interview ended at 0756 hours. <br />
