Melnyk #251 Approved By: <br />Date of Report: 07/2/2018 Date: 07/03/2018 <br />Page 1 of 4 <br /> <br /> <br /> <br />CASE NAME: TAYLOR, DONADONY BLAKE <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) <br />TYPE: INITIAL RESPONSE – HOSPITAL SCENE <br />AGENCY: SANTA ANA DEPARTMENT <br /> <br />On Monday, July 2, 2018, at approximately 0009 hours, Orange County District Attorney <br />(OCDA) Supervising Investigator STAN BERRY contacted me regarding an officer involved <br />shooting incident involving the Santa Ana Police Department (SAPD). BERRY requested I respond <br />to the SAPD Station assist with the investigation. <br /> At approximately 0121 hours, I arrived at 1020 West Civic Center Drive and met with <br />investigators from the OCDA, SAPD, Orange County Crime Lab (OCCL) and Orange County <br />Coroner’s Office (OCCO) who were present to assist in the investigation. <br />At 0206 hours, SAPD Sergeant ABEL OROPEZA #2499 conducted a briefing of the known <br />circumstances surrounding the incident for the personnel involved in the investigation. After the <br />briefing, BERRY requested SAPD Detective GUSTO CAPACETE and I conduct the hospital scene <br />investigation and witness interviews at the Orange County Global Medical Center (OCGMC), 1001 <br />North Tustin Avenue, Santa Ana. CAPACETE and I conducted the following audio digital recorded <br />interviews: <br /> Care Ambulance, Emergency Medical Technician <br /> Registered Nurse, OCGMC <br /> Trauma Surgeon, OCGMC <br />ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE <br />BUREAU OF INVESTIGATION <br /> <br />SUPPLEMENTAL REPORT <br />
