SA 18-021 <br />Investigator: D. Melnyk #251 Approved By: <br />Date of Report: 07/2/2018 Date: 07/03/2018 <br />Page 2 of 4 <br />For details regarding the statements provided during these interviews, refer to the <br />supplemental interview reports and digital audio recordings for each subject. <br />The scene was located inside Trauma Room 1 at OCGMC. Trauma Room 1 was identified <br />by a placard outside the door which indicated “T-1.” The room was being guarded by SAPD <br />Officer BLAKE KELLY #3378. <br />Orange County Crime Lab Forensic Specialist LENORA PENA-SANCHEZ arrived to <br />document the hospital scene. PENA-SANCHEZ took nineteen (19) digital photographs of the <br />scene under FR# 18-49132. Coroner Investigator PAUL HOAG processed TAYLOR’s body under <br />OCCL Case #18-03011-HO. <br />The body of DONADONY TAYLOR was lying supine on a bed in the middle of the room with <br />his head towards the west and his feet towards the east. TAYLOR’s lower torso was covered by <br />a white hospital sheet and blanket. <br />During a visual examination of TAYLOR’s body the following items were observed: <br /> Apparent gunshot wound near the sternum. <br /> Apparent gunshot wound to the left upper arm and left arm pit. <br /> (5) electro-cardiogram (EKG) leads and pads on the torso. <br /> White hospital identification bracelet on the right wrist. <br /> Red hospital identification bracelet on the right wrist. <br /> Handcuff attached to the right wrist. <br /> Intravenous (IV) line in the right forearm. <br /> Blood pressure cuff on the right arm. <br /> Intubation tube and harness on the head. <br /> Chest tube in the left chest. <br /> Pulse oximeter lead attached to left ring finger. <br /> Thoracostomy needles in the left and right upper chest. <br /> (2) Automated External Defibrillator (AED) pads on the front torso.
