OCDA Personnel:Toth, Thomas 366 <br />Involvement:Reporting 4/10/2023 <br />(OCFA) paramedics and Care Ambulence personnel provided medical care at the scene and then transported <br />MELGAREJO to Orange County Global Medical Center (OCGMC), 1001 N Tustin Ave, Santa Ana. <br />At approximately 23:25 hours, SAPD Detective BONILLA and I arrived at OCGMC and made contact with SAPD <br />Officers DARREN BLAND and DAVID TAROY, who were stationed outside of Patient Room #420, preserving the <br />hospital scene and controlling access to MELGAREJO. <br />The hospital scene was in OCGMC Patient Room #420. The Room was identified by a black placard displaying <br />"ROOM 420" affixed to the west wall of the door opening. The opening to the room had a solid door that was open. <br />There was also a blue patterned privacy curtain that was partially open. <br />On April 8, 2023, at approximately 00:50 hours, OCCL Forensic Specialist STEPHANIE ESPINOZA entered the <br />room and began processing the scene. <br />The following personnel and I were also within the scene during processing: <br />•RENE BONILLA, Detective, Santa Ana Police Department <br />•JORGE LOPEZ, Sergeant, Santa Ana Police Department <br />• Registered Nurse, Orange County Global Medical Center <br />Upon entering the room, I saw MELGAREJO lying supine on a bed, which was positioned perpendicular to the south <br />wall of the room. MELGAREJO'S head was toward the east and he was covered with a white blanket. He was <br />wearing a green patterned hospital gown and had a brown sock on his left foot. MELGAREJO was conscious, but <br />had been given pain medication approximately thirty (30) minutes prior to processing. <br />I observed MELGAREJO to have the following injuries and medical equipment attached to his person: <br />•Bandaging on left arm and left hand <br />•Bandaging on right upper thigh <br />•Bandaging on right lower leg <br />•Wound closed with metal staples on left upper thigh <br />•Pulse oximeter on right index finger <br />•White hospital bracelet on right wrist <br />•Intravenous (IV) line with tubing on right forearm <br />OCCL Forensic Specialist STEPHANIE ESPINOZA collected the following items of evidence under OCCL FR #23- <br />043030: <br />•Bullet in plastic container (Located and recovered from MELGAREJO'S rectum area by OCGMC Trauma <br />Surgeon ) <br />•MELGAREJO'S Clothing items (One white shoe, one white sock, one red bandana) <br />•DNA reference swabs/Buccals from MELGAREJO <br />Forensic Specialist ESPINOZA documented the scene by taking thirty-eight (38) digital color photographs under <br />OCCL <br />FR #23-043030. <br />ESPINOZA concluded hospital scene processing at approximately 01:15 hours. <br />Narrative By: Toth, Thomas, 366 NO Audio Recording : N/A <br />OCDA 23-001104.011 Report by Toth, Thomas 366 Page 3 of 4
