Agreement #W7-GJC-10 <br />City of Santa Ana <br />EXHIBIT A-1 <br />TO WIA COST REIMBURSEMENT AGREEMENT <br />GREEN JOBS CORPS <br />AUGUST 1, 2009-MARCH 31, 2011 <br />GENERAL PROGRAM REQUIREMENTS <br />D. Attainment of Recognized Certificate/Diploma/Degree: The goals for high <br />school diploma/GED attainment will be measured through academic <br />transcripts, academic advising forms, and copies of diploma/GED. <br />Completion of industry-identified soft skills programs will be measured <br />through attendance records and copies of certificates of achievement. <br />E. Literacy/Numeracy Gains: For out-of-school youth who are basic skills <br />deficient, attainment of prescribed increases in literary and numeracy <br />proficiency will be measured through pre- and post-assessments. <br />F. Retained or Returned to Secondary School (for at least two semesters): <br />This goal will be measured through academic transcripts and/or <br />appropriate academic advising forms. <br />*Summer Youth Employment Program participants will be required to complete a <br />work readiness component. (See Exhibit C-1). <br />V. PERFORMANCE STANDARDS <br />A. Contractor shall meet or exceed required federal, State and local <br />standards, measurements and outcomes of all funding streams included in <br />this Agreement and subsequent updates. Failure to meet performance <br />levels may be grounds for funding de-obligation. <br />B. Contractor shall adhere to WIA Customer Satisfaction Policy for <br />Participants and Employers per OCWIB Information Notices No. 04- <br />OCWDA-22, OCWDA-04-16 and any subsequent updates. <br />C. Contractor shall ensure that participant activities remain uninterrupted in <br />accordance with service delivery requirements of TEGL 17-05, and any <br />subsequent updates. <br />VI. SYSTEM SECURITY AND CONFIDENTIALITY <br />Contractor agrees to the following: <br />A. That all applications and individual records related to services provided <br />under this Agreement, including eligibility for services, enrollment, and <br />referral, shall be confidential and shall not be open to examination for any <br />purpose not directly connected with the delivery of such services. <br />B. That the sharing of individual and client information necessary for <br />provision of services under this Agreement, i.e.: assessment; universal <br />intake; program or training referral; job development, placement or follow- <br />up activities; and other services as needed for employment or program <br />support purposes, constitutes a valid use of such information. <br />WIA Cost Reimbursement Agreement - Green Jobs Corps Exhibit A-1 <br />Page 4 of 8
