DeGraffenreid Approved By: <br />Date of Report: 7-3-13 Date: 07/09/2013 <br />Page 1 of 2 <br /> <br /> <br /> <br />CASE NAME: GONZALEZ, JESSICA <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) <br />INTERVIEW OF: , TRAUMA SURGEON <br />UNIVERSITY OF CALIFORNIA, IRVINE- MEDICAL CENTER <br />101 THE CITY DRIVE SOUTH <br />ORANGE, CALIFORNIA 92868 <br /> <br /> <br />DATE AND TIME: JUNE 28, 2013 1103 -1115 HOURS <br />LOCATION: 101 THE CITY DRIVE SOUTH, ORANGE, CA 92868 <br /> <br /> On the above date and time, I conducted an audio digital recorded interview of Trauma <br />Surgeon, , in reference to a Santa Ana Police Department (SAPD) officer <br />involved shooting incident involving JESSICA GONZALEZ. has been a surgeon for seven <br />years and is employed by the University of California, Irvine, Medical Center (UCIMC). <br />was an on-duty trauma surgeon working a twenty-four hour shift beginning, June 23, 2013, at <br />0730 hours and ending, June 24, 2012 at 0730 hours. <br />I provided with a copy of UCIMC’S Emergency Department’s Flow Sheet for <br />patient ISOLATE DOE #9066488, dated June 23, 2013. also referred to his own “trauma <br />run” notes from his computer during the interview. Patient ISOLATE DOE has now been <br />positively identified as JESSICA GONZALEZ.
