EXHIBIT 2 <br /> Eligibility <br /> Participants must be enrolled and in good standing with the program to be eligible to receive <br /> incentives. <br /> A participant is considered in good standing when they: <br /> • Are actively participating in program services <br /> • Maintain communication with staff <br /> • Complete scheduled activities <br /> Participants who are unsuccessfully exited,non-responsive,or dismissed from the program are not <br /> eligible for incentives. <br /> Eligible Milestones <br /> Incentives may be issued for successful completion of the following verified achievements: <br /> Program Engagement <br /> • Completion of enrollment and orientation process, <br /> • Completion of any assessments,pre surveys as required by the program <br /> • Completion of Individualized Employment Plan <br /> Employment Achievement <br /> • Obtaining unsubsidized employment <br /> Continued Verification of Employment <br /> • Providing continued proof of employment based on contract guidelines and success <br /> measures <br /> Education&Training <br /> • Completion of secondary education (e.g., High School Diploma or GED) <br /> • Completion of an approved occupational training or certification program <br /> Program Completion <br /> • Completion of the program exit interview (includes completion of any updating of <br /> documents, surveys assessments) <br />
