Per the Finance Department: <br /> ■ The Employment Readiness Manager is the only authoritative body to submit requests <br /> for more cards. <br /> ■ The purchase of all cards may be delayed if the gift card log and case note & supporting <br /> document have not been completed by the Employment Readiness <br /> Specialist/Coordinator. <br /> Distribution of Cards to the Employment Readiness Specialist/Coordinator: <br /> ■ Requested cards will be hand delivered to each Employment Readiness <br /> Specialist/Coordinator. <br /> ■ Upon delivery, the Employment Readiness Specialist/Coordinator must sign a "received <br /> receipt" to receive gift cards. Delivery will be rescheduled if the Support Specialist is <br /> unavailable. <br /> ■ Received receipt will be kept for accounting records (Name, date, signature) <br /> ■ Azucena will upload backups to the folder in Teams labeled "STAFF—Gift Cards Copies <br /> 2023." <br /> o Rename the file as follows, Date given_Card Name_ Employment Readiness <br /> Specialist/Coordinator first & last name_# of cards given (easier access) <br /> 0 2023.02.28_Shell Gas Card—Jane Doe_(7) <br /> Distribution to Trainees: <br /> The Employment Readiness Specialist/Coordinator will provide trainees with a gas card/gift <br /> card, or bus passes upon eligibility, based on financial need. <br /> Transportation assistance is generally limited to one bus pass per month or up to two gas <br /> cards per month during the training phase, based on documented participation and financial <br /> need. Additional assistance outside of these limits may only be provided in special <br /> circumstances or emergencies with approval from the Employment Readiness Manager and <br /> appropriate documentation in Apricot. <br /> The Employment Readiness Specialist/Coordinator will make a copy of the gas card/ buss <br /> pass with a trainee signature of receipt and date given. The trainee must also acknowledge <br /> receipt by signing a receipt form confirming the item was received, the date issued, and the <br /> value/type of support provided. <br /> ■ The Employment Readiness Specialist/Coordinator will enter a case note in Apricot and <br /> upload the signed copy. <br /> ■ The Employment Readiness Specialist/Coordinator will upload backups to the folder in <br /> Teams labeled "TRAINEE—Gift Cards Copies 2022-2023." <br /> ■ Rename the file as follows, trainee's first & last name—Card Name—Date given (easier <br /> access) <br /> 8 <br />
