<br /> <br /> <br /> <br /> <br /> ARTICLE XI <br /> <br /> 11.0 EMPLOYEE INSURANCE <br /> <br /> <br /> 11.1 Health Insurance. The City shall contribute the following amounts toward the payment <br /> of premiums for affected employees and their dependents under the existing health <br /> insurance programs or new program or programs providing substantially similar or <br /> improved coverage and benefits selected in accordance with procedures in effect on the <br /> effective date of this Agreement: <br /> <br /> A. For each such employee who is covered under a spouse's non-City sponsored <br /> health plan, the City will pay the employee a cash payment each month in an <br /> amount equal to 50% of the monthly premium amount for the City's lowest <br /> "employee-only" coverage, if said employee waives, in writing, City-paid <br /> coverage. <br /> <br /> B. Effective January 1, 2008, January 1, 2009, and January 1, 2010,_respectively, the <br /> City shall contribute toward medical premiums an amount consistent with the <br /> rates then in effect for each tier of the Ca1PERS Kaiser Permanente HMO Other <br /> Southern California plan. <br /> <br /> C. Effective January 1, 2011 the City shall contribute toward medical premiums an <br /> amount consistent with the rates then in effect for each tier of the lowest priced <br /> available Ca1PERS approved HMO medical plan for the Other Southern <br /> California Region plan. Additionally, employees hired after January 1, 2012 shall <br /> also reimburse the City for the purchase of medical insurance through the City <br /> through a payroll deduction in an amount equal to the lesser of ten percent (10%) <br /> of the value of the rates then in effect for each tier of the lowest priced available <br /> CalPERS approved HMO medical plan for the Other Southern California Region <br /> plan, or $50 per month. <br /> <br /> E. Any contribution necessary to maintain benefits under said medical plans in <br /> excess of the amount set forth above shall be borne entirely by the employee. <br /> <br /> 11.2 Dental Insurance. Effective January 1, 2007, the City contribution amount will be <br /> increased to a maximum of up to ninety ($90) dollars per month per employee. This <br /> contribution amount of ninety ($90) dollars per month per employee shall remain in <br /> effect during the term of the Agreement, which expires June 30, 2013. <br /> <br /> Any contribution necessary to maintain benefits under said dental plans in excess of the <br /> amount set forth above shall be borne entirely by the employee. <br /> <br /> 11.3 Long-Term Disability (L.T.D.) Insurance. Effective January 1, 2002, for the term of this <br /> Agreement, the City shall contribute up to a maximum of ten dollars and thirty-nine cents <br /> ($10.39) per month to employees enrolled in the 130 day elimination period LTD plan. <br /> <br /> <br /> 50 <br /> 25F-52 <br />
