Use space below if needed) <br />Department: PWA <br />Phone/Ext.: <br />Signature: <br />.r Ira �:if �zHf �z�l l': <br />Revised 10.31-12 <br />Note: <br />On December 5, 201 1 the City Council approved Crosstown Agreement Amendment 2 (Item <br />25E Council Action & Minutes - attached), to increase the amount of contract from $50,000 to <br />$75,000 and extend the contract to June 30, 2012. All work on the contract amendment was <br />completed and contractor paid in full by the end of the contract. However, after the <br />completion of the contract, it was discovered that Amendment 2 was not executed. <br />City Attorney Office has determined that it is not necessary at this time to pursue agreement <br />execution since the contract has been long completed and the contractor has been paid in <br />full. <br />
