Evidence of Completion [24 CFR 983.155(b)] <br />At a minimum, the owner must submit the following evidence of completion to SAHA in the <br />form and manner required by SAHA: <br />~ Owner certification that the work has been completed in accordance with HQS and all <br />requirements of the Agreement; and <br />~ Owner certification that the owner has complied with labor standards and equal opportunity <br />requirements in development of the housing. <br />At SAHA's discretion, the Agreement may specify additional documentation that must be <br />submitted by the owner as evidence of housing completion. <br />SAHA will determine the need for the owner to submit additional documentation as evidence of <br />housing completion on a case-by-case basis depending on the nature of the PBV project. SAHA <br />will specify any additional documentation requirements in the Agreement to enter into HAP <br />contract. <br />SAHA Acceptance of Completed Units [24 CFR 983.156] <br />Upon notice from the owner that the housing is completed, SAHA will inspect to determine if <br />the housing has been completed in accordance with the Agreement, including compliance with <br />HQS and any additional requirements imposed under the Agreement. SAHA will also determine <br />if the owner has submitted all required evidence of completion. <br />If the work has not been completed in accordance with the Agreement, SAHA will not enter into <br />the HAP contract. <br />If SAHA determines the work has been completed in accordance with the Agreement and that <br />the owner has submitted all required evidence of completion, SAHA will submit the HAP <br />contract for execution by the owner and must then execute the HAP contract. <br />PART V: HOUSING ASSISTANCE PAYMENTS CONTRACT (HAP) <br />17-V.A. OVERVIEW <br />SAHA must enter into a HAP contract with an owner for units that are receiving PBV assistance. <br />The purpose of the HAP contract is to provide housing assistance payments for eligible families. <br />Housing assistance is paid for contract units leased and occupied by eligible families during the <br />HAP contract term. The HAP contract must be in the form required by HUD [24 CFR 983.202] . <br />17-V.B. HAP CONTRACT REQUIREMENTS <br />Contract Information [24 CFR 983.203, FR Notice 11/24/08] <br />The HAP contract must specify the following information: <br />~ The total number of contract units by number of bedrooms; <br />~ The project's name, street address, city or county, state and zip code, block and lot number <br />(if known), and any other information necessary to clearly identify the site and the building; <br />ii/29/io Page 17-13 <br />
