Contractor <br />You are hereby directed to make the herein changes from the plans and specifications or do the following described work not included in the plans and <br />specifications on this contract. <br />NOTE: THIS CHANGE ORDER IS NOT EFFECTIVE UNTIL APPROVED BY THE CITY COUNCIL OR CITY MANAGER. <br />Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time <br />Change requested by <br />PUBLIC WORKS AGENCY <br />EXTRA WORK AT TIME AND MATERIALS <br />1. Install bulkhead to seal leach line to prevent the flooding of the excavation for the SA -04 <br />precast concrete vault, on a time and materials basis, as directed per PCO #6 issued on <br />October 6, 2011. <br />FINAL COST = $4,467 <br />2. Repair shopping cart anti -theft system as directed per PCO #7 issued on November 23, <br />2012. <br />AGREED COST = $1,925 <br />3. Compensation for delay costs including loss of production, stand -by time, and remobilization <br />incurred during the relocation of the SA -04 precast concrete vault. <br />AGREED COST = $11,424 <br />4. Pothole to determine utility locations for the storm drain relocation on the northwest corner <br />of Bristol Street and Warner Avenue, on a time and materials basis, as directed on August <br />22, 2011. <br />FINAL COST = $5,336 <br />Page 1 of 2 <br />
