The service intake database system determines the client's eligibility,
name, address, case facts, advice provided and other important statistical information. Additionally,
SAHLOP maintains paper case files which document all the work completed on behalf of each homeless
person. The project also documents and maintains all required forms to substantiate that the homeless
individual "resides" in Santa Ana.
The Supervising Attorney of the project keeps track of all individuals assisted through the project. Case
lists are generated on a quarterly basis and all matters are reviewed by the Supervising Attorney with the
assigned staff paralegal to ensure proper and adquate services are provided to the homeless individual.
This review process also effectively contributes to closing cases in a timely manner.
The close monitoring of cases and maintenance of statistical information helps SAHLOP to evaluate this
program. LASOC evaluates services on a regular basis by assessing individuals' needs, special
circumstances and legal issues that require resolution. SAHLOP will deliver services that reflect the
current needs of the homeless.
Exhibit A -1
