City of Santa Ana <br />Scope of Work <br />Name of Organization Women's Transitional Living Center, Inc. (WTLC) <br />Name of Funded Program Emergency Shelter Programs <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />670 Persons <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12 -month <br />contract period. <br />228 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />The Emergency Shelters Program includes the 45 -day shelter, 90 -day (substance abuse treatment) shelter, as <br />well as the Step -Two Transitional shelter (up to two year stay) and the Safety Net emergency motel shelter. The <br />primary goals of the program are identification of victims of domestic violence, intervention in both the immediate <br />and intergenerational cycls of violence and other high -risk issues associated with domestic violence (e.g., <br />substance abuse, mental illness, gang involvement, child abuse) and stabilization of those high -risk issues. Self - <br />sufficiency is considered to have occurred when victims find transitional /permenant (preferrably permanent) <br />housing, obtain employment, cease substance abuse, begin the process (if needed) of documentation, live free <br />from abuse, and become emotionally stable. <br />Services will include, but not be limited to: shelter, food, clothing, toys, case management, counseling, 24/7 <br />hotline, translation, transportation, social service /legal /immigration advocacy, on -site medical screening and <br />care, educational /empowerment/process groups, substance abuse treatment, career assistance (counseling, <br />resume preparation, interview techniques, work experience /placement, GED assistance), therapeutic children's <br />programs, housing /financial assistance, safety /independence planning, and household establishment <br />assistance. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
