' : City of Santa Ana <br />I%Wior <br />Any calls for service that are not handled by the Contractor through the First Tier <br />or Mutual Aid Service, that require OCFA to request service from another <br />ambulance service operator, will result in the assessment of a "Turned Call" <br />penalty in the amount of $1,000 per occurrence. Contractor is to immediately <br />notify the OCFA EMS Chief of said occurrence within 24 hours. Failure to report <br />within 24 hours will result in an additional penalty of $500. <br />ii.Late Report/Late Payment Penalties <br />Any reports, fees, or reimbursements that are required to be submitted to the <br />OCFA Contractor pursuant to the Contract Documents (e.g., Quarterly Response <br />Time Reports, CQI, etc.) and that are not submitted on time may result in the <br />imposition of a $500 penalty per occurrence. <br />C. Corrective Action Plan <br />Situations which come to the attention of, or are reported to, the OCFA EMS Section <br />Battalion Chief and which appear to constitute a Contractor service or performance <br />deficiency or substantial inadequacy, as determined by the OCFA, shall be immediately <br />investigated by the OCFA. An example of such situation might be the Contractor's <br />failure to achieve at least a 90% response time performance in any single Code category <br />for a quarterly reporting period. At the discretion of the OCFA, a Corrective Action Plan <br />may be imposed on the Contractor to correct identified and verified performance <br />deficiencies and inadequacies. The OCFA authorized representative shall meet to develop <br />a written Corrective Action Plan (CAP) within fifteen (15) working days of the <br />identification and verification of the service or performance deficiency, or substantial <br />inadequacy, in accordance with the following CAP requirements: <br />i.CAP Format <br />The CAP shall describe the following: <br />(1) The service or performance deficiency, or substantial inadequacy shall be <br />identified; and <br />(2) The method by which Contractor is to correct the service or performance <br />deficiency, or substantial inadequacy, shall be outlined.
