It aims at <br />Fridays <br />enhancing students' cognitive, language, motor and social development. <br />8:30 am — 1:00 pm <br />After School Arts <br />The ASA for Life program provides artistic training in the fields of <br />Monday — Friday <br />Program <br />Music, Visual Arts, Dance, Musical Theatre, and Technology training to <br />3:30 pm — 8:30 pm <br />at -risk youth, and children with disabilities in the community. <br />And Saturdays <br />9:00 am — 5:00 pm <br />Therapeutic Arts <br />This program serves children and youth with disabilities of all ages and <br />Monday — Friday <br />Program <br />special needs, including: learning disability, health impairments, physical <br />3:30 pm — 8:30 pm <br />disability, developmental disabilities and others. <br />Saturdays <br />9:00 am — 5:00 pm <br />Learning Academy <br />This program provides homework support and tutoring to students (ages <br />Monday — Friday <br />& Tutoring Program <br />6 — 18 yrs) in Language Arts (reading and writing), math, and other <br />4:00 — 6:00 pm <br />academic subjects. <br />Saturdays <br />9:00 am — 5:00 pm <br />OCCTAC <br />This program provides outreach, prevention, referrals and engagement <br />Various days /times <br />Family Wellness <br />with individuals at -risk of mental health problems. Other support <br />throughout the week <br />Program <br />services include parenting, counseling, and support groups. <br />16. Describe how the program interacts and is supported by other (proposer's) agency programs. <br />OCCTAC has a great collaborative relationship with all agencies from the Youth Service Provider Network, <br />including the Santa Ana Work Center. Also, OCCTAC has had co- enrollments with Taller San Jose, <br />Conservation Corps., Orangewood Foundation, Cambodian Family, and SAUSD. In addition, OCCTAC is <br />providing all agency partners, the opportunity to enroll their participants in our certificate programs offered at <br />our agency, as well as co- enroll high school drop -out students with SIATech to earn their high school degree. <br />17. Describe what supportive services participants will receive. WIA participants receive a wide variety of <br />support services at OCCTAC. These include individual mentoring, individual counseling, support groups, <br />leadership training, academic support as needed, tuition support for certificate /vocational programs, or other <br />related academic programs, books for related course work, work related supplies, portfolios, tutoring supplies, <br />bus passes, gas cards, and finger printing and TB tests are covered for work related requirements. <br />18. Describe how services will be made easily accessible to youth in Santa Ana. OCCTAC has long <br />operational hours (8:30 am — 8:30 pm), and therefore, we can accommodate the different schedules of WIA <br />youth. We are open in the evenings, and also in the weekends. In addition, OCCTAC's facility is strategically <br />located close to bus terminals for the convenience of our families. There are many SAUSD schools in the <br />vicinity, as well as easy access to public transportation. Students can take the bus or in some instances walk to <br />the Center. Also, if needed, participants will be provided with bus passes and /or gas cards to facilitate access to <br />training and work experience. In summary, our hours of operation, location, and wide variety of programs make <br />our services very accessible to youth in Santa Ana. <br />19. Describe the services that will be offered to the youth participant during the 12 month follow- up <br />period after exit? Indicate how participants will attain performance measures during the follow -up <br />period. The OCCTAC staff will provide 12 -month follow -up services for "exited" participants. OCCTAC is <br />committed to empowering transitional age youth with and without disabilities to reach their highest potential in <br />life. WIA participants that exit the program will be encouraged to continue to participate in monthly Life Skills <br />Training, monthly Support groups, individual counseling, as well as well as any other support services they may <br />be in need of OCCTAC staff will be available to support participants, whenever needed. <br />Exhibit A <br />
