Project 08 -1700 <br />Request for Approval of Change Order No. 4 <br />March 17, 2014 <br />Page 2 of 4 <br />FINAL COST = $6,950.19 <br />3. During excavation for the 36" storm drain, an 18" sewer main was encountered that <br />was not shown on the plans. The sewer main runs diagonally across the trench, this <br />caused the contractor ineffiencies, required additional shoring, pipe supports and <br />hand excavation. (PCO #29) <br />FINAL COST = $8,156.02 <br />4. During excavation for the 12" PVC water main an AT &T duct bank was encountered <br />in the trench line. The duct bank is 84" to the bottom, with the top being 36" from the <br />proposed finished surface. These elevations do not permit the new 12" water main to <br />be installed over the top of the duct bank and therefore must be installed under it. A <br />vertical offset must be constructed to enable the installation of the proposed water <br />mains under the AT &T duct bank. (PCO #31) <br />FINAL COST $2,410.13 <br />5. To establish the connection between the new 8" PVC water main and the existing <br />water system on Santa Ana Blvd. west of Bristol, the water supply to the adjacent <br />medical clinic needed to be turned off. To accomplish this without disrupting the <br />clinic's activities, the work needed to be done at night. This compensates the <br />contractor for the added payroll premium for night work. (PCO #34) <br />FINAL COST $2,997.22 <br />EXTRA WORK AT AGREED PRICE <br />6. After the start of construction, revised plans for the north east corner of Bristol and <br />Civic Center were issued. The revisions call for the relocation of an existing pole <br />mounted area light on private property which will necessitate trenching, intercepting <br />the existing conduit, removing the old foundation and placing a new foundation. (PCO <br />#26) <br />AGREED PRICE $3,949.41 <br />7. After the start of construction, revised irrigation and landscaping plans for the east <br />side of Bristol between Santa Ana Blvd. and Civic Center Dr. were issued. The <br />revisions call for reducing the irrigation and landscaping in two areas, for which a <br />credit was issued, and adding irrigation and landscaping in third for which an <br />additional cost was incurred. This cost represents the difference between the credit <br />and the additional costs. (PCO #33 & 35) <br />AGREED PRICE = $4,302.11 <br />EMERGENCY WORK <br />8. During a heavy rain storm on February 28, 2014, staff responded to several resident <br />concerns about excessive flooding in the intersection of Avalon Avenue and <br />Ponderosa Street. Staff investigated the concerns and determined that a street <br />culvert was blocked by tree roots. Several unsuccessful attempts were made to <br />
