The case file will include, but not be limited <br />to, the following: <br />4.13.lInitial referral form. <br />4.13.2 Documentation of all correspondence in regards <br />to the participant's participation in VTR or WEX activities, <br />including any correspondence involving any subcontractors. <br />4.13.3 Participant attendance records. <br />4.13.4 Documentation, including dates, of any problem <br />occurrences reported by the VTR or WEX work site. <br />4.13.5All correspondence related to anv Workers' <br />Compensation injury. <br />4.14 Develop relationships with local businesses by <br />networking and developing work sites for WEX participants. <br />4.15 Develop appropriate training and WEX sites in the <br />community, monitor participant's attendance, and communicate <br />participant and attendance issues with WTW staff, as determined <br />by ADMINISTRATOR. <br />4.16 Attend meetings as requested by ADMINISTRATOR. <br />(WAM0313) 7 of 22 <br />(April 8, 2013) <br />25B-57
