HSIPL- 5063(141) <br />Subject: Change Order No. 1 <br />Contractor: PTM General Engineering Services, Inc. <br />5942 Acorn Street <br />P, <br />Riverside, CA 92504'' <br />Project Description: Traffic Signal Modification on McFadden at Lyon -, <br />Change Order Number: 1 Total This Change Order $ 1,5091 <br />Accounting Unit - Activity Number: 05917660 - 12676002028 <br />JUSTIFICATION <br />The traffic signal modifications at McFadden Avenue and Lyon Street project <br />provides for the installation of a phased left -turn signal which includes upgrading the <br />existing signals and enhancements to the existing signage at this intersection. This <br />will allow staff to communicate with traffic signals, make traffic signal timing <br />adjustments as necessary based on real time traffic conditions and enhance service <br />and increase safety at this location. <br />After potholing for utilities, as required by the contract, for the new traffic signal poles, <br />as shown on plans, significant conflicts were discovered with existing underground <br />utilities preventing the installation of the traffic signal pole at the southeast corner of <br />the intersection. In order to avoid the underground utilities the traffic signal pole was <br />moved 8 -feet to the back of walk which is beyond the 5 -foot state limit for a pole <br />mounted pedestrian push button (PPB) from the crosswalk. Therefore, Traffic <br />Engineering requested the installation of a post mounted PPB to meet this state <br />mandated requirement. The estimated additional cost will cover the procurement and <br />installation of a working PPB. (PCO No.1) A prior approval was granted for this work <br />on June 25, 2013. <br />AGREED COST = $1,500 / <br />
