Project 13 -7516 <br />Request for Approval of Change Order No. 1 <br />July 29, 2013 <br />Page 2 of 3 <br />3. At the Garfield and Brown Street Parking Lot along the northern boundary of the site, <br />adjacent to an existing block wall on private property, the plans call for removal of an <br />existing PCC slab. During the removal of the slab it was discovered that the block <br />wall was constructed on top of the slab without a footing. As this creates an unsafe <br />condition, it is recommended to remove the wall. Consequently, removal of the wall <br />creates an opening in the perimeter of the parking lot. The Community Development <br />Agency has requested that 150 LF of 6' high wrought iron fence be constructed <br />along the entire north boundary of the project in lieu of the existing and proposed <br />block wall. The block wall removal will be done at contract unit prices. (PCO #04) A <br />prior approval was granted for this work on July 15, 2013. <br />AGREED COST $ 9,750 <br />4. At Madison Park, the plans call to construct a CMU tool shed with a prefabricated <br />roof. The plans designate a manufacturer and model number of the roof. (Icon <br />Shelter System #SQ10S -P6) This model number does not match the dimensions of <br />the tool shed shown on plan. Consequently, a larger prefabricated roof (Icon Shelter <br />System #SQ12S -P6) must be provided and installed. (PCO #05) A prior approval <br />was granted for this work on July 26, 2013. <br />AGREED COST $ 1,728 <br />5. At Madison Park, the plans show the proposed chain link fence in the same location <br />as the proposed basketball court light poles. To negate the conflict, custom brackets <br />need to be fabricated to enable using the proposed light poles as fence posts. (PCO <br />#06) A prior approval was granted for this work on July 26, 2013. <br />AGREED COST $ 468 <br />6. At Madison Park the plans call to provide and install three basketball poles with <br />backboards and nets. The plans designate a manufacturer and model numbers. <br />After the start of construction, the Parks, Recreation and Community Services <br />Agency requested alternate pole, backboard and net models be installed. (PCO #07) <br />A prior approval was granted for this work on July 26, 2013. <br />AGREED COST $ 5620 <br />7. At Mabury Park, the plans indicate that the existing electrical service point will be <br />utilized to provide power for the proposed lights being installed under this contract. <br />After the start of construction, Southern California Edison (SCE) indicated that the <br />existing service was inadequate for the load of the new lights. Consequently a new <br />electrical service point must be established with 200 LF of 3" conduit and wire. (PCO <br />410) A prior approval was granted for this work on July 26, 2013. <br />AGREED COST $ 13,041 <br />
