12 <br />oversight and conduct spot-checking of files to ensure that case notes are up-to-date. The Case <br />Manager will meet with the Director at least monthly to evaluate the program's successes and <br />challenges, review Individual Service Plans, track outcomes, and review W/O/R/K Center client <br />reports. The Program Director will review satisfaction surveys and implement necessary program <br />changes with the Case Manager. <br />Continuous Improvement: The CareerWorks Program Director will develop a satisfaction <br />survey for the program. The Case Manager will administer quarterly satisfaction surveys. The <br />Director will analyze findings and create a quality improvement plan based on surveys. The Case <br />Manager will implement continuous improvement as guided by analysis of the satisfaction surveys. <br />XII. Prior Outcomes <br />Not applicable. <br />VIII. Facility <br />Location and Hours of Operation: The Career Works Program services will be based out of <br />the KidWorks Dan Donahue Center (DDC) located at 1902 W. Chestnut Avenue (main cross <br />streets: First Street and Raitt Street), while some meetings may take place at the KidWorks <br />Center on Townsend (KCT) at 817 S. Townsend Street (cross streets: Raitt Street and McFadden <br />Avenue). DDC is open during non-traditional hours from 8am — 7pm (Monday -Thursday), 8am- <br />9pm (Friday) and 9:30am-lpm (Saturday). KCT is open from 3pm-7pm (M -F) but is available <br />outside of these hours, as needed. <br />Youth -friendliness and Accessibility: The youth at KidWorks are active in decorating the center <br />and arranging the space. At DDC, the main place of operations, there is a tutoring lab, activity <br />room, recreation room, conference room, multi-purpose room, and computer lab with 20 <br />computers, 20 iPads, and printers. DDC has open and closed spaces for workshops and <br />counseling. The services are in Santa Ana to serve the city's residents and the facility is <br />compliant with the Americans with Disability Act. <br />Computers and Internet Access: KidWorks' facility is equipped with wireless, high-speed <br />internet access. All agency staff have their own workspace and computer with Microsoft <br />Windows 7, Vista, or XP operating systems. Staff use a variety of computer applications <br />including Microsoft Office and Access. <br />XIV. Fiscal Capacity <br />KidWorks operates with a staff of 37 with 22 full-time positions and 15 part-time positions. <br />KidWorks is also currently a placement site for 14 AmeriCorps members and interns. Since <br />incorporating as a non-profit in 2002, KidWorks has established itself as a strong, stable <br />community-based organization funded by more than 100 foundations and over 1,500 individual <br />donors. KidWorks' financial stability is not dependent on WIA funds. <br />EXHIBIT A <br />
