Professional Counseling to assist with personal, relationship and /or family <br />concerns <br />• Career/ Vocational Assessment and Counseling <br />• Additionally, staff consultation and support regarding customer issues and <br />problem management <br />The Professional Counseling is provided to address personal concerns ranging from <br />job loss grief, low self - esteem and ego strength/ coping skills, to stress management <br />difficulty, relationship /family stress, to clinical depression and anxiety. <br />• Educational, supportive and cognitive - behavior clinical interventions <br />• Individual counseling that may involve single or multiple appointments over <br />time in a confidential setting <br />• Couples or family counseling when identified as potentially helpful <br />• Group counseling or workshops may be conducted according to the need <br />and commonality of concerns <br />• Referral to relevant professionals, agencies, and resources as needed <br />The Vocational Assessment and Counseling to assist in determining appropriateness <br />of or readiness for vocational training, what career areas, and training and /or <br />employability issues. <br />• , A two -day process involving administration of a multiple battery of <br />assessment instruments related to vocational interests, work values, <br />personality type, abilities/ aptitudes, basic skills (the TABE), and transferable <br />skills along with a separate individual interpretation /counseling <br />consultation <br />• A formal written report for referring case manager along with consultation <br />• Basic Skills testing using the TABE for prescreening or retesting <br />• Career counseling for customers with uncertain goal direction <br />Fees for <br />Hourly fee: $45 (except for vocational assessments) <br />Services: <br />Vocational Assessments charge: <br />15 or more (total) per calendar month: $200 - Full; $160 - Modified <br />14 or less (total) per calendar month: $225 - Full; $175 - Modified <br />The fees and charges for all services would not exceed $90,000 in a fiscal year. <br />EXHIBIT A Page 1 of 3 <br />
