APPENDIX A Proposal Response Form <br />(Concluded) <br />Sample Job #2: <br />Scope of Work: Contractor shall provide and install one (12/6) -multi -mode / single mode, <br />fiber optic cable running from the basement Equipment Room ERB.2 to the 8th floor <br />Telecommunications Closet TC8.1. Fiber optic cable will terminate in ERB.2 onto an <br />appropriately configured CommScope SYSTIMAX LGX Fiber Optic Distribution System, <br />using an LST1 U -072/7 shelf with ST -type connectors. In TC8.1, fiber optic cable should <br />terminate onto an appropriately configured CommScope SYSTIMAX Sliding Combination <br />Shelf, using a 600132 One -Unit High type shelf with ST -type connectors. Rack space <br />required exists at both termination points. One, empty 4 -inch conduit exists between <br />ERB.2 and TCB.1 (the Basement Telecommunications Closet that is vertically stacked <br />with the rest of the floors including our destination closet, TC8.1). Contractor shall provide <br />and install (4) one inch, non - plenum, corrugated innerduct from ERB.2 to TCB.1 via <br />existing 4 -inch conduit noted above which is 150 ft in length. At exit point of conduit, <br />transition (1) of (4) installed innerduct to (1) one inch, riser - rated, corrugated innerduct, <br />which will traverse from the basement, stacked Telecommunications Closet, TCB.1, up to <br />TC8.1. Contractor shall provide (1) 4 -inch core hole between TCB.1 and TC1.1. Building <br />Facilities Management has determined that floor is clear of obstruction for 4 -inch core. <br />Core hole shall be sleeved, and fire stopped per local, state, and industry standard codes. <br />1) Provide a breakdown of hours to be billed. <br />Innerduct and Fiber optic Cable Placement — 24 hours <br />Lout and Sleeve for one core fire stop riser — 10.3 hours <br />Terminate, Install Housings. Testing and Labeling — 15,7 hours <br />Total -- 50 Hours <br />2) Provide a breakdown of cost of materials. <br />• 400' Fiber optic cable 12/6 f12- strand 50un mm / 6- strand single- mode: $689.60 <br />Note: Minimum purchase of composite fiber optic cable is 1640' <br />• 1240' Fiber optic cable 12/6 f12- strand 50um mm ( 5- strand single -mode for stock: $2,137.66 <br />• 800' Riser Rated Innerduct based on (4) runs x 150' plusU run of 200' to rout <br />riser from TC8.1 up to TC8.1 $547.02 <br />• Termination Material: $1,331.92 <br />• Sleeve and Fire Stop Material: $369.2.8 <br />• Total Material: $5,075.78 <br />• Sales Tax: 406 -05 <br />3) Provide details and amounts of any and all other charges that would be billed to the <br />City. <br />Sub - contract for concrete coring services $1,233.33 <br />TOTAL Cost of Project including Labor, Materials and sub - contract: $9,200.11 <br />4) Provide approximation of elapsed time to complete work, beginning from authorization <br />Page 8 of 9 <br />
